Juxtarenal aortic aneurysm (JRAA) is an unusual, but not rare, pattern of aneurysmal disease of the abdominal aorta in which dilatation extends up to, but does not involve, the renal arteries. The objective of this report was to retrospectively analyze experience with JRAA repair at a tertiary referral center over a 5-year period.

In some cases of juxtarenal abdominal aortic aneurysm (AAA), the paravisceral segment is not suitable for a branched endograft due to space restrictions. The creation of a proximal landing zone using a thoracic tube graft in combination with a Fenestrated Anaconda device (Terumo Aortic) implanted distally may represent a feasible treatment option.

A RelayPlus stent graft was used to create a proximal landing zone for a Fenestrated Anaconda device in a patient with a juxtarenal AAA, penetrating aortic ulcer, and dilatation of the descending thoracic aorta. Endovascular aneurysm repair of the infrarenal or thoracic aorta has been shown to be a less invasive alternative to open surgery. A combined aneurysm of the thoracic and abdominal aorta is complex and challenging; the involvement of renal and/or visceral branches requires new treatment methods. A hybrid approach is currently an accepted alternative to conventional surgery.

Despite advances in endovascular aneurysm repair, open repair requiring suprarenal aortic cross-clamping is still the treatment of choice for JAA. The majority of abdominal aortic aneurysm (>80%) are infrarenal in nature for which both endovascular and open reconstruction options exist.
